Hexarelin is a synthetic hexapeptide (6 amino acids) belonging to the growth hormone‑releasing peptide (GHRP) family and is a stabilized modified analogue of GHRP‑6. As a highly effective agonist of the growth hormone secretagogue receptor (GHS‑R), Hexarelin acts at both the pituitary and hypothalamic levels to stimulate pulsatile endogenous growth hormone (GH) release via activation of GHS‑R1a. The presence of a 2‑methyltryptophan (2‑Me‑Trp) substitution in its structure confers greater stability and more sustained GH‑releasing activity compared to the parent compound GHRP‑6. Beyond its classical GH‑releasing function, Hexarelin also exhibits non‑GHSR‑dependent cardioprotective effects: it binds to the CD36 receptor, improves cardiac contractility, reduces myocardial ischemia–reperfusion injury, and inhibits myocardial fibrosis.
